What Are Blood Cancers?

Blood cancers are cancers that affect the blood, bone marrow, and lymphatic system, which together play a crucial role in immunity, oxygen transport, and overall body health. Unlike solid tumours that form in organs, blood cancers originate in the cells that produce blood or support immune function. These cancers interfere with the body’s ability to fight infections, carry oxygen, and control bleeding, which is why their symptoms can be widespread and sometimes difficult to recognise early.

Types

Blood cancers are broadly classified into three main groups: leukemia, lymphoma, and multiple myeloma. Each type affects different blood or immune cells and behaves differently. Leukemia begins in the bone marrow and leads to the production of abnormal white blood cells that crowd out healthy cells. Lymphoma develops in the lymphatic system, which includes lymph nodes, spleen, and lymphatic vessels, and affects lymphocytes, a type of white blood cell. Multiple myeloma arises from plasma cells, another type of white blood cell responsible for producing antibodies, and primarily affects the bone marrow. In addition to these, there are several rare subtypes and related blood disorders that are also considered blood cancers.

Risk Factors

Blood cancers can affect people of all ages, from children to older adults. Some types are more common in childhood, while others are more frequently seen in adults or the elderly. The causes of blood cancers are often complex and not always clearly identifiable. In many patients, no single cause is found.

However, certain factors are known to increase risk. These include genetic predisposition, exposure to high levels of radiation, prolonged exposure to certain chemicals such as benzene, previous cancer treatments involving radiation or chemotherapy, and some inherited or acquired immune system disorders. Certain viral infections have also been linked to an increased risk of specific blood cancers. It is important to note that having risk factors does not mean a person will definitely develop blood cancer, and many people diagnosed have no known risk factors at all.

Symptoms

The symptoms of blood cancers can vary widely depending on the type and stage of the disease. Early symptoms are often vague and may resemble common infections or general health issues, which can lead to delays in diagnosis. Common warning signs include persistent fatigue, weakness, and shortness of breath due to low red blood cell counts (anaemia). Frequent infections or infections that are slow to resolve may occur because abnormal white blood cells cannot function properly. Easy bruising or bleeding, frequent nosebleeds, or bleeding gums may result from low platelet counts.

Some patients experience unexplained weight loss, loss of appetite, night sweats, or prolonged fever. Swollen lymph nodes in the neck, underarms, or groin may be noticed, especially in lymphoma. Bone pain, back pain, or repeated fractures can occur in conditions affecting the bone marrow, such as multiple myeloma.

Because blood cancers affect systems throughout the body, symptoms can involve multiple organs at once. Some people may notice an enlarged spleen or liver, which can cause a feeling of fullness or discomfort in the abdomen. Others may experience numbness, tingling, or kidney-related symptoms in certain types of blood cancer. The wide range of possible symptoms means that blood cancers can be difficult to recognise without medical evaluation.

Diagnosis

Diagnosis of blood cancer typically begins with a detailed medical history and physical examination. The doctor will ask about symptoms, their duration, any history of infections, exposure to radiation or chemicals, family history of cancer, and overall health. A physical examination may include checking for swollen lymph nodes, enlarged liver or spleen, and signs of anaemia or bleeding.

Blood tests play a central role in diagnosing blood cancers. A complete blood count (CBC) can reveal abnormalities in red blood cells, white blood cells, or platelets that raise suspicion. Additional blood tests may assess kidney function, calcium levels, protein levels, or markers of inflammation. While blood tests provide important clues, they are usually not sufficient on their own to confirm a diagnosis.

A bone marrow examination is often required to establish the diagnosis. This involves collecting a small sample of bone marrow, usually from the hip bone, and examining it under a microscope to identify abnormal cells. In cases of suspected lymphoma, a biopsy of an enlarged lymph node may be performed. Imaging tests such as X-rays, ultrasound, CT scans, MRI, or PET scans may be used to assess lymph nodes, bones, or organs and to understand the extent of disease.

Once blood cancer is confirmed, further tests are often performed to classify the specific type and understand how the disease is behaving. This may include genetic and molecular testing of cancer cells, which helps doctors better understand the nature of the disease.

Importance Of Early Detection

Blood cancers are complex conditions, but early recognition of symptoms and timely medical evaluation are key to achieving better outcomes. Persistent fatigue, frequent infections, unexplained bleeding, or swelling of lymph nodes should never be ignored. Awareness and early diagnosis play a vital role in managing blood cancers and improving the quality of life for patients.
